3SL Dental membership terms
- 50% OFF tooth whitening (home tooth whitening kits/trays)for all new adult members in addition to free initial check up
- All fees are due on the day of treatment
- FREE childrens treatment upto 6 years old with adult on members plan
- All patients must have had a full examination at 3SL Dental prior to joining the membership plan
- Payment is by monthly Direct Debit
- If the Direct Debit is cancelled within 1 year then full treatment fees will need to be paid retrospectively for benefits already received
- Terms and conditions are reviewed every 6 months and are subject to change
- To be able to offer these fantastic benefits, membership is strictly limited and benefits can be revised at any time
Cancelling your membership
If you find you ever wish to end the agreement you can do so simply by giving us one month’s notice. Naturally, if amounts due to us are not paid for two months in a row we may terminate the arrangement.
